The short answer
Health insurance usually covers the medical and clinical parts of treatment, such as detox, therapy, psychiatric care and medication, when they are medically necessary. It usually does not cover the luxury parts: private suites, chefs, spa services or ocean views.
So with luxury rehab, insurance often pays for some of the care, and you pay the rest.
PPO, HMO and Medi-Cal
| PPO plans | HMO / Medi-Cal | |
|---|---|---|
| Out-of-network care: | often partly covered | rarely covered |
| Luxury programs: | sometimes in-network for clinical care | usually not covered |
| Pre-approval: | often required | required, in-network only |
Plans vary widely. Always check your own plan.
How to check your coverage
Call your insurer
Ask about substance use treatment benefits, deductibles and out-of-network coverage.
Get a verification of benefits
Ask the program to run a written verification of benefits (VOB) for your plan.
Get the estimate in writing
Ask what you will owe out of pocket, and what happens if the insurer denies days.
Is addiction treatment an essential health benefit?
Yes. Under the Affordable Care Act, mental health and substance use disorder services are essential health benefits for individual and small-group plans.
What if my claim is denied?
You can appeal. Ask the program for the clinical records that support medical necessity, and ask your insurer for the reason in writing.
Does insurance cover at-home detox?
Coverage for in-home medical detox varies by plan. Ask the at-home provider and your insurer before you start.
Will my employer see my claims?
No. Your employer does not see your medical claims, and treatment records are protected by HIPAA and 42 CFR Part 2.
